We are seeking people with the right values, people who are positive and caring, who are able to contribute and make a difference to people’s lives, who have good communication skills and are able to work as part of a professional multi skilled team.
Our Support at Home service works with many professionals in the community, such as Social Workers, District Nurses, and Occupational Therapists to support people’s choice to live at home. We support people with differing needs that may be a combination of physical, sensory or mental health needs to achieve what matters to them. The service also supports short-term rehabilitation programmes in the community helping people to regain lost skills or learn new ways to maximise their independence.
